Harbor Health Medicare Advantage Members Gain Access to New Care Model for Complex Medical Needs; First-Time Access for Non-Medicare Insurance Exchange Members Aged 50+
(firmenpresse) - DETROIT, MI -- (Marketwired) -- 04/11/16 -- , the leading provider of healthcare services for patients with complex medical needs, will be offering Harbor Health Plan, Inc.''s Medicare and Insurance Exchange members ages 50+ in Metro Detroit access to ConcertoHealth''s individualized health care experience, beginning April 11, 2016.
This is the first time non-Medicare Insurance Exchange patients enrolled through Harbor Health Plan aged 50 and above have the opportunity to experience the patient-focused care provided by ConcertoHealth before they are eligible for Medicare coverage.
ConcertoHealth has more than a decade of experience caring for Medicare, Medicaid and complex medical needs patients. The company integrates clinical care with social services to ensure that patients and their families receive the individualized support they need to improve health outcomes and peace of mind.
