However, medication mismanagement, whether due to undesirable drug interaction, miscommunication between doctors and patients, or noncompliance, lands many older adults in the hospital each year.
To help older adults better manage their medications, decreasing their chances of hospitalization due to medication mismanagement, Kendal Northern Ohio created "Tips for Better Medication Management: At the Doctor's Office, At the Pharmacy, and At Home." []

"Oftentimes, an older adult is seeing four, five, or six physicians," Dave Zientarski, Kendal at Home director of care coordination, says. "It's impossible for [the physicians] to know all the medications you are taking without [the physicians] being physically there when you're taking them." Older adults must be prepared to take an active role in their medication treatment plan to help reduce their risk of medication mismanagement.
