It is often difficult to fully anticipate requirements during product development. Great development engineers and scientists are often "dreamers" who excel in creative environments. Trying to force them to operate within the confines of structured stage-gate and quality processes such as DFSS can be challenging. We find it crucial to funnel projects through a formal scale-up process that requires multiple successful runs to establish a quality control plan and processing conditions. The people involved in scale-up are the experts in this area and their job is to convert a promising new concept into a commercially viable product. For big launches of new technology, structured processes such as PPAP, used by the automotive industry, are very good at improving initial quality. Incorporating failure mode analysis for both the actual design as well as the manufacturing process can significantly reduce defects.
Buy-in and commitment from the top level of the organization. If you have that, everything else is manageable. For a gate process to work, executives must make meetings a priority and plan their schedules accordingly.
